Contact Tracing & Health Declarations

Victorian Government public health requirements mandate that Surrey Park Swimming record the contact details of all attendees to the centre for contact tracing in the event of an outbreak of COVID-19.

For the health and safety of our staff, customers, members and vistors; all attendees at our sites are also required to complete a health declaration to ensure that they are not an unreasonably high risk of carrying the COVID-19 virus, as a condition of entry.

The above information must be provided on each visit and can be completed online by clicking on our Contact Tracing and Health Declaration form below. Forms must only be completed at the time of entry.

The information on this form will only be provided to government authorities on request, all private information related to this form will be deleted 21 days after completion.

COVID Safe Officer

Surrey Park Swimming have appointed a COVID Safe Officer to oversee the requirements of WorkSafe Victoria and Swimming Australia.
